Group Tour Form
This form enables us to process your directions more simply. Please choose File->Print and print out the form, then print legibly when filling it out. We should receive this form within 3 weeks of the event in order to ensure mailing your admittance bands in time. You must register in advance to receive the discounts.
When we receive this form along with your check, we will then mail your admittance bands (and pins if applicable) back to you at the address below to your contact person's attention. This way, there are no lines to combat when your group arrives. Please note: the coach driver and/or tour guide are admitted free. Special admission bands will be waiting for them at the door.
Please have your contact person let us know when you have arrived at Quilt Odyssey so we can welcome you and present your group with their programs. Our "no fail" process guarantees that, should someone fail to show up in your tour group, all you need to do is present their unused admittance bands (and pins if applicable) to our staff upon arrival and we will refund you by check after the show.
